1) Older printers have a tunsgten heating element to warm up the fuser. Took a while. Newer ones use a flash lamp. The fuser melts the loose toner powder into the paper to make the image permanent.
2) Worn pickup "D" shaped roller(s), too much dust on "D" roller, paper tray not adjusted properly, paper damaged by humitity, problems with the transmission, broken or damaged paper tray (they get dropped sometimes)
3) The paper was probably too late in getting to a critical sensor. Feed rollers may be too old and have lost the ability to move the paper. Time for a mainteance kit?
4) All machines that process paper jam sometimes. If it keeps happening look for remnants of a previous jam still lurking in the paper path. Damaged fuser rollers are also frequent culprits.
5) The separation pad, usually soft urethane rubbber, sometimes cork, is worn or dirty. This pad holds the second sheet of paper from being picked up. Again, this will happen occasionally to the best of printers. Paper is another factor here.
6) Printing the same page continuously? Dunno. Have not seen this problem. Could be the formatter, which takes the code sent over from the computer and makes up a dot pattern that will look like what you wanted. Could also be a corrupted driver (the software that works with the printer), a software problem. Eye-Dee-Ten-Tee problems (ID10T) with the user?

Get the handbook for your printer and many of these questions may be answered for your particular machine. Most can be downloaded from the Internet. The really inexpensive bottom of the line printers often do not have a service manual available since they are considered disposable.
