Lines every 3.75" on HP4m+

Hi:
I installed a new maintenance kit and toner cartridge, which cleared up the original problems. Now I have about one inch lines starting at the left side of the paper spaced every 3.75 inches from top to bottom.

What else do I need to do?

Are you using refilled toner cartridges? It could be your toner.
